For Dubai passport holders (Category A), the card is valid for five years and requires submission of an electronic copy of the Emirates ID, passport, family book, a recent personal photo, a medical report detailing the disability, and a people of determination card if issued by the Ministry of Community Development.

Decree holders (Category B) also receive the card for five years, needing an Emirates ID or decree certificate, a personal photo, a medical report, a people of determination card if applicable, and proof of residence in Dubai.

Citizens from other emirates (Category C) can obtain the card for five years by providing an Emirates ID, passport, personal photo, medical report, people of determination card if applicable, and proof of residence.

Expatriate residents (Category D) must submit an Emirates ID, passport, residence permit, personal photo, medical report, people of determination card if applicable, and proof of residence.

Tourists (Category E) can apply for the card valid for their stay, requiring a visit visa, passport, a medical report or disability card from their home country, proof of residence, and a personal photo.
